Information We Collect
- Contact form: name, email, optional phone number, and your message.
- Orders: name, email, optional phone, optional order note, selected products, strengths, quantities, and payment method (card, Venmo, or Zelle).
- Card payments: processed by Stripe on its secure checkout. Stripe collects your card details, email, and shipping address. We do not receive or store full card numbers.
- AI wellness guide: the goals you type are sent to an AI service to generate suggestions. The text is not saved by our website or kept by the AI service after your suggestions are returned.
- Client portal: account email, profile contact information, shipping address, order history, payment status, consultation requests, clinic responses, and account sign-in records.
- Stored on your device: your cart contents and whether you dismissed the "install app" prompt (see our Cookie Policy).
Health-Related Information
Messages, order notes, and AI guide entries may contain sensitive health information if you choose to include it. Please share only what is needed; detailed medical history is best discussed during your consultation. Website submissions are delivered by email and are not part of a medical record unless your provider adds them.
[CONFIRM / LEGAL REVIEW] Whether JZ Advanced Wellness is a HIPAA covered entity, whether website submissions are treated as protected health information, whether business associate agreements exist with email, payment, or AI providers, and applicable state health privacy laws (e.g., New Jersey and the states you ship to). We do not currently claim HIPAA compliance for this website.
How We Use Information
- To respond to inquiries and schedule consultations.
- To process, confirm, and ship orders and send order confirmations and receipts.
- To provide AI-generated educational suggestions.
- To operate the secure client portal, coordinate consultations, and let authorized clinic staff manage orders and requests.
We do not sell your personal information. We do not use advertising or analytics trackers on this website.
Service Providers
- Stripe — card payment processing and tax calculation.
- Google (Gmail) — delivery of form submissions and order emails.
- Website hosting and email sending providers.
- An AI model provider — processing of AI wellness guide text.
- Google Fonts — font files loaded from Google servers, which receive your IP address.
- Venmo / Zelle — only if you choose to pay with them, under their own privacy policies.
Retention
We keep client information only for as long as you are our client. When your relationship with the practice ends, your information is deleted. Payment records held by Stripe may be kept as required for tax and accounting purposes.
Your Choices
You may request access to or deletion of information you submitted by emailing jzurndorfernp@gmail.com. Because we keep client information only while you are a client, deletion requests are honored when your care with the practice ends.
Security
Data is transmitted over encrypted connections (HTTPS). No method of transmission or storage is completely secure.
Children
This website is not intended for anyone under 18.
